Your employee benefits now include mandatory CI coverage of $10,000. You are covered for 31 Critical Illnesses.
Optional CI – Employee Paid, coverage may be elected at any time.
Coverage starting at $5,000 on yourself, spouse and eligible dependents. No medical application is needed.
For amount greater the $5,000 the medical application is required and the coverage maximum available is $100,000 on yourself and spouse and $25,000 on eligible dependent children.
